The 27-year-old makes the switch to Paisley after leaving Rangers and can also count Newcastle United and Port Vale among his former clubs.
Manager Jim Goodwin told the club website: "It was well-documented that it was a position we needed to fill and Jak was one of our top targets. Vaclav Hladky left here with everybody’s best wishes but I think we have brought in a very able replacement in Jak."
